Instructions for Transferring Securities or Funds

from another financial institution to the

Unity Church Restoration Fund

 

 

 

Please use the attached form to transfer securities or funds from a brokerage, mutual fund, insurance company, or other financial institution to the Unity Church Restoration Fund account at Charles Schwab & Co.  The account name, number, and Tax ID are preprinted on the form.

 

If you have any questions regarding required information, please contact a Schwab representative at (800)-435-4000.  Information, including status update of transfers, is available at www.schwab.com.
